Poly-pharmacy issues are extremely common, harmful and costly.  Doctors of Pharmacy are in an ideal position to reconcile medications between physicians, or when patients transition from home, between facilities, to rehab, and back home. 

Patient education can improve adherence and outcomes dramatically. We are available to your patients one-on-one to help them understand their condition and medications. We also can teach them how to use home testing equipment.
